Instructions for Completing this Form:
 
  • This application form needs to be completed in one session.
  • You may need up to 30 minutes. It cannot be saved and reopened once commenced.
  • It is recommended that you read through the form and gather the required information before commencing. The enrolment form will be considered as signed once it is submitted. Original signatures will be required on future documentation should your application be successful.
  • Acceptance is subject to an interview with the Principal when spaces are being filled. An application does not guarantee enrolment.
  • Please read the Contract of Enrolment and other policies on the Citipointe College website.
  • A non-refundable application fee of $132 must be paid on application before the form will be processed. Payment options are at the end of the form.
